Ok guys first a quick system details:
      Windows 7
      iTunes 10.4.1.10
      iPhone 3GS
      iOS 4.3.5

The problem - I can't get my photos to sync

I've got a folder for it on my pc, loads of pics on the iPhone. Go to sync......nothing. Nada gets transferred to the pc.

I manually copy the photos across, create a folder, put some pics in it and re-sync. Now I get that folder on the iPhone as well as one called "Camera Roll" and one "Photo Library", both with different stuff in.

I take a photo and it puts it to the Camera Roll. I hook up to sync to get that photo on my pc....and nothing.

      I can't figure this.

How can I have one syncing to the other properly?

On your computer, make sure digital camera notification is enabled and the desired actions and programs are selected to handle your photo and video import.
Note: If you have a media application on your computer to which you would like your images and videos imported, you can select that program from the AutoPlay settings window in the Control Panel.
To import photos or video from your iPhone, iPad, or iPod touch to the Windows Photo Gallery:

      1. Connect the iPhone, iPad, or iPod touch to your computer. Windows should automatically open the AutoPlay dialog box.
Note: If the AutoPlay dialog box does not automatically open, you may need to enable the behavior.
      2. Select the "Import pictures and videos using Windows" option in the AutoPlay dialog.
      3. You can optionally choose to tag the photos and video you want to copy. Click Import after you're finished tagging.
      4. AutoPlay will then import your images and videos to the Windows Photo Gallery. This screen appears only while the import process is active:
      5. Once the import is complete, Windows Photo Gallery will launch with your photos and videos highlighted under the "Recently Imported" section:

      Great thing to know.

Do you maybe know how to copy other pictures from the phone to the computer? the ones that ware not taken by iPhone, but copied from another computer (my hard disk died, and I cant transfer the photos back on my computer cos iTunes wants to delete them)

      Try this:
      1. Click on the Start button
      2. In the 'Search Programs and Files' search box, type "Autoplay"
      3. Scroll to the bottom of the Autoplay page
      4. You should find a list of Apple Devices at the bottom of the list.
      5. Beside the name of your iPhone click the menu to the right and select: "Import pictures and videos using Windows"
      6. Click Save.
      7. Make sure your iPhone 4 isn't screen locked (enter your PIN if it is).
      8. Connect your iPhone USB cable to phone and PC.
It should now import your photos to your PC - probably to a folder called Pictures or MyPictures...
From now on Windows should open the 'Import Pictures and Videos Using Windows' option to let you import your pictures.

Basically, you want windows to recognise the iPhone as a seperate camera.
